Service Information

Requesting Records

The Cook County Medical Examiner’s Office maintains its full and complete records in accordance with the Illinois Local Records Act, 50 ILCS 205. 

If you are a law firm, insurance carrier, legal representative or authorized government agency/organization and you are requesting records from the Cook County Medical Examiner’s Office you are directed to submit your request through our secure portal. In the portal, you will be required to input information, upload documentation, and pay applicable fees. The release of records may require notarized documentation, certified court orders, or other forms. 

All records will be released securely for downloading at the discretion of the MEO, in accordance with valid laws, court orders, and our internal policies. Certified copies will be electronically delivered through our portal.

Records Available

In each case, the following records may be available and there are mandatory fees charged pursuant to 55 ILCS/4-7001 and Sections 32-1 and 38-144 of the Cook County Code of Ordinances as applicable: 

  • Autopsy Reports: $50.00

  • Toxicology Reports: $25.00

  • Miscellaneous Reports: $25.00

  • Photos: $5.00 per photo

  • X-rays: $10.00 per x-ray

  • Histology slides: $10.00 per slide

Subpoenas 

All subpoenas for records or testimony must be properly served in accordance with Illinois law. Electronic submission of a subpoena to the portal does not constitute formal service unless specifically authorized. 

Testimony

Requests for expert witness testimony by the Chief Medical Examiner, Deputy Chief Medical Examiner, and Assistant Medical Examiners must be coordinated in advance and fees for testimony are charged pursuant to 55 ILCS/4-7001 and Sections 32-1 and 38-144 of the Cook County Code of Ordinances as applicable.
